For everyone who may need to report wrongdoing. It lists the channels, including anonymous reporting, commits to acknowledgement within 7 days and feedback within 3 months, and prohibits retaliation, in an EU Directive 2019/1937 style.
- App: Attestation Flow for Confluence
- Category: Governance
- Edition: Picking the pack works in every edition; the paid settings and the starter text need a paid licence
- Re-acknowledge: every 365 days

Starter text
#Template. This template is a starting point, not legal advice. Adapt it to your organisation, your contracts and the law that applies to you, and have it reviewed by a qualified professional before you rely on it.
What you can report
Any suspected wrongdoing connected to [Company]: fraud, bribery, data protection breaches, health and safety dangers, environmental harm, harassment, or breaches of law or of our policies.
How to report
- Your manager or [compliance contact].
- [Anonymous reporting channel, link or phone].
- External authorities, where the law allows.
What happens next
We acknowledge your report within 7 days and give feedback on the follow up within 3 months. Reports are handled confidentially by [designated person or team].
Protection
Nobody who reports in good faith will suffer dismissal, demotion or any other retaliation. Retaliation is a serious disciplinary matter.
Quiz
#Includes 2 quiz questions; the answers are not published here. They are shown only in the app's requirement editor, to the person setting up the requirement, who can change them before saving.
- Can you report a concern anonymously?
- No
- Yes, through [anonymous channel]
- What happens to someone who retaliates against a whistleblower?
- Nothing
- They face disciplinary action
Use it
#Edit a Confluence page, insert the Attestation Flow macro, open the requirement editor and pick the pack under Start from a policy pack. It fills the note and the due date; with a paid licence it also fills the re-acknowledgement window, the minimum reading time and the quiz, and can add the starter text to the page. Required readers are never preset: you choose them.
